Abstract

Health insurers today function primarily as third-party administrators of health benefits for public and private payers, rather than as integrated stakeholder organizations capable of generating, assimilating, and leveraging claims data into actionable insight. This gap creates significant operational inefficiencies and constrains the competitive scope of traditional risk and revenue management services.

To address this, we propose an Integrated Payer Operations Model (IPOM), structured around distinct operational domains that enable holistic management of key performance (KP) data through a shared data infrastructure. These domains, in turn, support the development of business intelligence products and services aimed at optimizing community health outcomes and directing mitigation funding toward high-impact, low-income social determinants of health interventions.

The Claims Intelligence domain is used here to illustrate the model’s design and its resulting addressable market potential. Functioning as an analytical framework, Claims Intelligence enables exhaustive examination of all KP health services delivered to the insured population, allowing non-optimal resource utilization to be identified from the demand side across the full continuum of care.

Claims adjudication—the first step in processing a medical claim—determines coverage validity and the admissibility of the claimed amount. Whether managed internally or through external administration, disbursement of legitimate claims must be timely and accurate to preserve the insured population’s trust. Delayed or altered reimbursement signals a risk area that can undermine continuity of coverage and reduce member engagement. Developing Key Performance Indicators for Claims Adjudication and Integrity therefore serves a dual purpose: it establishes an evidence-based accountability standard for insurers and functions as a proactive income-assurance service supporting the management of health funds across private, corporate, and public-sector risk environments.
